Basically you register with social services and they allocate you a doctor and healthcentre, but you can choose which one you want, for example, we live in Caleta which is in the Antigua Ayumento and use the one in Antigua, but I have a neighbour who speaks little Spanish, and although there is a doctor in Antigua that speaks some English, he switched to the health centre in Rosario, because there is a doctor there that speaks good English
