I read on the DGT website that renewing your Spanish Driving Licence after age 65 is fairly simple, and so it proved!
You can start the process up to 3 months before the old one expires.
You choose one of the approved Medical Centres who will do your medical tests AND they will process all the paperwork for you! If you are 70 you only pay the cost of the medical, (45€) no tax and no fee for processing. The medical centre even does your photo! You don't need an appointment - just go in.
You need to take in your ID and existing Driving Licence.
If you complete your medical test OK (eyes and computer game thingamy) they give you a temporary paper licence while you wait for your new card to arrive. New card will be valid for 5 years.
This is one of the centres in Puerto (very nice staff: receptionist and doctor. Doc spoke quite good English.):
